If you mean in the first bar on page 22, where it says m.d in the 2nd stave down, I would ignore it and play the two treble clefs with your right, and stick to LH playing the bass, if that makes sense. I might be on about something completely different though!

The m.d seems rather silly though, if thats what you mean, it doesn't make things easy for yourself!
