You should contact the board and ask them. If you took the exam after 1992/93 (I can't remember the exact year now) they will have the results online to check up. If it is longer ago than that, then they ask for a certificate of proof of Grade 5 theory for Grade 6+ exams. Either way, they will be able to advise you. I guess the worst case scenario, if you really have lost the certificate and the board cannot verify you have G5 theory, is that they will ask you to resit it.

It will depend how much more than 10 years back you took the exam. If you took in or after 1994, ABRSM will confirm marks for free, if you give them certain details. See extract below from the regulations

k) On application, the result of a past examination may be confirmed in writing to a concerned party (e.g. an Applicant, candidate, parent/guardian):

1. Examinations taken in 1994 or later: the candidate’s name, the date and place of examination and the Applicant’s name (and Applicant Number) must be supplied. The total marks gained are confirmed in writing by ABRSM for no charge. On payment of a fee, a duplicate certificate is provided, if requested.
2. Examinations taken before 1994: a past results search form must be completed. A confirmation of marks fee is charged and a duplicate certificate is included, if requested.

The past results form is here:
http://www.abrsm.org/regions/fileadmin/use...earchForm09.pdf

If you justed wanted it for purposes of entering the higher practical grades, I think you only have to supply the certificate if the exam was taken in 1993 or earlier. See extract from regulations below:

d) Prerequisites for Practical Grades 6, 7 and 8: candidates must already have passed one of the qualifications listed in the Exam Entry section. This must have been passed before the closing date for the Practical examination and the certificate or mark form must be supplied at the time of entry, except for ABRSM qualifications gained in or after Period C 1993 and taken in the UK or Ireland (for which supporting documentation will only be required in cases where ABRSM is unable to verify the qualification from its records).

Hope this helps

If you need to prove you have G5 theory, (in the case the board cannot verify whether you have G5 or not) you have to post a copy of the certificate to the board separately. Online entry only allows you to enter for the exam. The board will verify whether you have G5 theory afterwards and if they can't verify it, they will send a letter to the applicant - you if you enter for the exam yourself - and ask for a copy of the certificate.

I hope you get a quick response from them.
