Well in my case I would have to go back to 1966 in my uni days at Nottingham when with the Unversity choir under the late Ivor Keys (what a great name for a musician, he had the chair of Music at Nottingham at the time and was a regular broadcaster mainly as an organist).
We did the Bach Magnificat, and new work by Tony Hewitt Jones , 'Good Company' a short cantata. This was a live broadcast on Radio 3 (I think called the BBC Third Programme at the time)
Not been involved in anything since.

Not as a performer, but as part of the audience

I've been in several Radio 3 live audiences north of the border, and I've also been caught (many years ago) in the audience at a Runrig concert that was broadcast on TV...
Most of these weren't strictly 'live' though, due to post-production editing and such like, but still

As a child I appeared with Atarah ben Tovin on stage in one of her Atarah's Band concerts.
....then a long interval of many years....
Until last year when I did an piano performance on BBC radio.
Neither event was strictly Live though. I can't remember when the Atarah thing was televised but my radio thing went out two hours later.

A choir I was in until I was 18 did some televised concerts and recordings for CD, but actually Live we did a recording in Lincoln Cathedral but I can't remember what radio station it was on!

Our choir does a live Nine Lessons and Carols just before Christmas each year. It's broadcast by Isles fm, but is picked up by several other local radio stations in the UK.

I
nearly did a live broadcast last year, presenting the highlights of the Western Isles Young Musician of the Year Competition, but as this was a first experience for me, I chickened out and pre-recorded it instead!

I was in a band that appeared in one of the very first DIY/renovate your house shows. It was in 1995/6 and was to celebrate the end result of a makeover of a barn in Suffolk. I think the programme was called Home Front or something similar. We were there all day but only about 5 minutes worth was shown.

I once played a live classical guitar duet with a young lady on Radio York's breakfast show. I think we played something by Fernando Sor. Apart from that only live radio interviews to do with Formula one, yachting, bowls, and local libraries. Anything else I've forgotten.

Yes and Yes again:
We were in a battle of the bands competition aired on ytv (yorkshire itv) when I was about 18. Funny thing was my stepfather who was then a new feature in our lives accompanied my Mum and got picked out of the audience as a "supporter" It was the first time he'd heard us !!!!

I remember him being asked what he thought and if he was an avid band supporter and his responses being "Yeah, they're all right !" And "something like that!!"
I've also played my sax on Minster Fm (Our local station) when I joined in the rather impressively named "James Watt Big Band" which was in actual fact .... A small group of local musicians who mucked in together to cobble together something fairly decent and "surprisingly Christmassy!!" for a number of light turn ons.
I've also been seen in the audience for the Evelyn Glennie Concert held at Leeds Boys Grammar school back in the 80s.

A televised Sunday morning service from the Methodist church I then attended - many many years ago - piano. I accompanied the Junior Choir.

None musical but...
I had the (mis?)fortunune to be cornered by a couple of radio interviewers and TV crews on the day I got my GCSE results - my excuse to what I said was that was totally hyper with the thrill of sucess...
Now, I dunno if I'd quite like a recording or not. I mean, it'd be nice to hold onto that day properly, as the memories are getting hazy after 5 years (eeep!!), but I don't think I could ever listen or watch without cringing.
Especially if anyone tracks down my rant about people who say GCSEs are getting easier...

Not live...but I was interviewed in November for a section of the Radio 4 prog 'Last Word'. It was recorded on the Thursday and broadcast on the Friday.

I appeared for about 5 seconds in the sports section of ATV news in 1977.
I phoned Capital radio phone-ins twice in 1980-82: Once concerning the Flowers report into London Medical Schools (I was against) and once concerning smoking on the Tube - London Underground had just banned it (or were thinking of it, I forget which) and there was something of an uproar of protest (I was in favour of the ban).
I had a request on Classic FM the day before my grade 8 piano exam 18 months ago - they played a Chopin Ballade for my wife but she missed it.
Not what you'd call a high media profile!

Our local radio station asked people to phone in if they could play a pop song on a musical instrument. I was 17 at the time and phoned in and played "Children" (Robert Miles). I won that day's contest (but then was only up against someone playing "Wonderwall" on the pots and pans!!) and got entered into a draw for a holiday to Majorca. 1 in 5 chance of winning - I didn't win

I've also been on our local BBC news in my scientist role as they were talking about something sciency...there's me holding a blood sample, pretending to analyse it

Yes - an NYO prom in about 1980 and a Radio 3 lunchtime concert from Broadcasting House in about 1982.
It's all been downhill since then
